This is the standardized form used by the Faculty Personnel Committee to collect course evaluations as described in Chapter V, Part A.2. of the Faculty Handbook. Faculty are not to supply this form to students. Instead, faculty should follow the course evaluation instructions provided by email about two weeks before the end of classes.
SELF-REPORT FORM
Faculty being evaluated for retention, tenure, promotion, and merit are asked to fill out a self-report form. A copy of the self-report form, as extracted from the 2013-2014 Faculty Handbook, is provided for reference below. Starting in 2012, faculty are now asked to complete this form online. Faculty are asked to follow the instructions in the email requesting a self-report.
PEER EVALUATION FORM
