• FERPA Information Release
  OWU is committed to protecting your privacy and complying with the Federal Education Rights and Privacy Act (FERPA). Please use the “FERPA Information Release” link to allow us to share your information with specific individuals.
• Application for Transfer Credit (PDF)
  Use this form to take courses at another institution and transfer the credits back to OWU. This form is required to transfer courses back to OWU.
• Transfer Credit Conversion Guide (PDF)
  This form converts OWU units into semester or quarter hours. The form also converts semester or quarter hours to OWU’s unit system.
• How to Request an Enrollment Verification Letter
• Petition for Special Permission (PDF)
  Complete this form to petition the Academic Status Committee for special permission. Please note the petition must have all required signatures before it will be reviewed. Petitions are reviewed every Monday at 4:10 p.m. during the academic year.
• Change-of-Schedule Form (PDF)
  Use this form to add, drop, or withdraw from a course.
• Audit Application (PDF)
  Complete this form to audit a course. Please note that you will not officially be registered for the course and will not have a transcript.
• Election of Credit/No Entry (PDF)
  Complete this form is to take a course credit/no entry.
• Change of Address (PDF)
  Use this form to officially change your permanent address in our system. You may submit this form to the Office of the Registrar.
• Application for Return to OWU (PDF)
  Former students not currently enrolled at OWU who wish to return should complete this form. Students currently taking a leave of absence must complete this form as well. Applications deadlines are as follows: Fall – July 1, Spring – November 15, and Summer – April 15.
• Application for Departure (PDF)
  Complete this form either to take a leave of absence or terminate enrollment at the University.
• Appeal of Academic Dismissal
  This form is only available to students dismissed during the current semester. If you were dismissed in a prior semester, please use the Application for Return to OWU (PDF).
