Bishop ACCESS Application

In order to complete the Bishop ACCESS application process, students will need to submit the following:

• Application
• Documentation of disability that impacts executive functioning (including any diagnostic or evaluation reports)
  • Documentation may include, but is not limited to, the most recent IEP/504 with specific diagnosis listed and/or most recent psycho-educational evaluation. You can find additional information regarding documentation on the Accessibility Services webpage.
  • Please contact the Accessibility Services Office directly with questions regarding documentation requirements.
